[Surgery of supracondylar humerus fractures in children].
To analyse the clinical results of supracondylar humerus fractures (SCHF) surgically treated in our institution. We conducted a prospective study on 60 children (29 girls and 31 boys) with SCHF who were surgically treated between October 1997 and November 2002. Surgery was done at an average of 8.4 days postinjury. Our surgical indication was based on clinical and radiographic results of closed reduction. Out of 60 patients, 43 had type-II (15 had a type-II according to Gartland and Werley classification) and 17 had type-III (4 had type-III). The average follow-up was 38 months. Lateral X-rays of upper limbs were done at 5, 10, 15, 20 and 30 days postoperatively and then monthly. Incisions were sutured only if they showed signs of infection. Compartment syndromes did not occur. In 12 patients (20%), of the 30 closed fractures, an open reduction was performed. These were mainly type-II fractures and open injuries. At the 15th postoperative day, 63% of the fractures were reduced. The final radiographic results showed 35 type-I (58%), 9 type-II (15%) and 12 type-III (20%). Our study has shown that, although the reduction of the fracture is the most important key in treating fractures, the reduction should be made early and closed fractures treated conservatively might have better clinical results..
